Ayahuasca and Antidepressants: The Interaction That Matters Most
This is the one interaction that has actually hurt people. Here is the mechanism, the timeline, and the rule that matters more than any of it.

If you take an antidepressant and you are considering an ayahuasca ceremony, this is the page to read carefully before any other. Most of what is written about ayahuasca focuses on the experience. This is about chemistry, and it is the part that has actually caused harm.
The mechanism, in one paragraph
Ayahuasca is a brew of two plants. The vine, Banisteriopsis caapi, contributes compounds called β-carbolines — harmine, harmaline, tetrahydroharmine. A second plant, usually chacruna, contributes DMT, the compound responsible for the visionary effects. Taken by mouth on its own, DMT would do nothing: an enzyme in your body called MAO breaks it down before it can take effect. The β-carbolines inhibit that enzyme, which is what lets the DMT become activeRuffell 2023
Here is the consequence that matters. Inhibiting MAO does two things at once: it lets the DMT work, which is the point — and it leaves other substances that this enzyme would normally break down without their brakes. The risk is not in the DMT. It is in what the MAOI effect lets through.
Which medications this applies to
This list is not exhaustive, and it is not a substitute for your doctor checking your specific case against your specific historyMAPS 1995
- SSRIs — fluoxetine (Prozac), sertraline (Zoloft), paroxetine (Paxil), escitalopram, citalopram, and others.
- SNRIs — venlafaxine, duloxetine.
- Tricyclic antidepressants.
- Pharmaceutical MAOIs — phenelzine, tranylcypromine, moclobemide. Combining two MAOIs is especially dangerous.
- Lithium and antipsychotics.
And because the MAOI effect leaves other substances without their brakes, this second list matters just as muchMAPS 1995: tramadol and pethidine/meperidine; dextromethorphan, found in many over-the-counter cough syrups; St John's Wort, a "natural" mood supplement that is nonetheless serotonergic; decongestants and cold and flu remedies containing pseudoephedrine or ephedrine; asthma inhalers; diet pills; stimulants including amphetamines, methylphenidate, cocaine, MDMA and mescaline; and levodopa, dopamine, carbamazepine and certain antihypertensives.
The practical version of that list: write down everything you take — prescription, over-the-counter, supplements, sleep teas, protein powder, whatever it is — and bring it to your doctor. If you are unsure about something, the default answer is ask first, not try it and see.
The honest nuance most guides leave out
We are going to give you the qualification too, because a warning you can verify is worth more than one you have to take on faith.
Research notes that serotonin syndrome from ayahuasca alone has only been observed at doses far higher than ceremonial ones, and that there is no direct evidence linking ayahuasca to this syndrome under normal useRuffell 2023 That is real, and it is worth knowing.
But it changes completely once you add an antidepressant to the equation. In fact, the one case of serious harm from combining ayahuasca with another substance that is documented in the literature is precisely a case of serotonin syndrome from fluoxetine plus ayahuascaSystematic review 2020 This is not a theoretical concern. It is the concrete one.
How long a washout actually takes

Tricyclics also require their own margin, and there is no single figure that applies to every case — it depends on the specific drug and on your history. Which is the point: none of this is arithmetic you can do at home.
Never stop an antidepressant on your own so you can take ayahuasca.
Stopping an antidepressant without supervision is dangerous in itself, regardless of ayahuasca: it can trigger withdrawal symptoms, relapse, and crisis. Whether to stop, how, and over how much time is an exclusively medical decision that needs weeks or months of planning.
If that means postponing your ceremony by six months, postpone it six months. The medicine has been there for centuries; it will be there next year.
The outcome nobody prepares you for
There is a real possibility worth naming: the right answer for you may be not to stop the medication at all, and therefore not to do ayahuasca right now.
That is not a failure. If you are on an antidepressant that is working, trading it for a ceremony is a bad trade, and any honest source should tell you so. A guide that only ever walks people toward the ceremony is not a guide, it is a sales funnel.
What to do this week
- Write the full list of everything you take, including supplements and over-the-counter medicines.
- Book the doctor's appointment now, not at the end of your preparation. If a washout is involved, the clock needs weeks of margin, and that clock starts the day you ask.
- Ask three specific questions: is there a contraindication in my case? Is there an interaction with my current medication? If an adjustment is appropriate, is it safe and advisable, over what timeframe, and under whose supervision?
You do not need your doctor to approve of your reasons. You need them to review your chemistry. That is medicine, not ideology, and any serious professional will understand the difference.
